London Regional Cancer Program Patient Assistance Program

Reduce patient out of pocket expenses such as wigs, drugs, equipment rentals, parking, etc. Funding is not available for living expenses, out of province travel for treatment, tests/procedures that are not funded through OHIP, or private pay chemotherapy

Eligibility

  • Emergency, short-term situations when funding from other sources and services is not available (expenses incurred within 6 months)

  • Adult who is undergoing treatment within the London Regional Cancer Program and/or its afflicated South Western Ontario regional partner sites

Trillium Drug Program

The TDP helps people not over the age of 65 who have high prescription drug costs relative to their household income. It covers all drugs listed under the Ontario Drug Benefit (ODB) program. 4% of your household's total net income (after your taxes) is your deductible which will be split into 4 equal payments over the year. You pay your deductible by purchasing ODB approved prescription drug products at your pharmacy, once these purchased produces equal the amount of your deductible, then the ODB pays for most of the cost of your drug. However there is a co-payment ($2) for each prescription that ODB covers. This program automatically enrolls every year after you have enrolled.

  • Valid Ontario health card

  • Resident of Ontario

  • Cannot receive financial help through Ontario Works or Ontario Disability Program

  • Cannot be a resident of a Long-Term Care home or Home for Special Care

  • Cannot receive Home Care Services

  • Cannot have private health insurance

Power of Hope: Cost Reduction Program

Multi-dimensional plan that offers financial assistance to qualifying cancer patients who are 40 years and younger, providing a reduction in the overall cost required to pursue fertility preservation treatment.

Female: reduced clinic fees of at least 33% for eg retrieval and egg/embryo cryo-preservation, reimbursement up to $1000, 1 year of storage.

Male: reimbursement for sperm cryo-preservation of up to $350, 1 year of storage

How to Apply

  • Application

  • Proof of insurance company that drugs and procedures are not covered

  • Confirmation form applicant's oncologist

Eligibility

  • Treatment must be part of a fertility preservation center part of the Power of Hope program (centres in BC, NB, ON, SK)

  • Individual income of $50 000 or less, or household income of $100 000 or less

  • Canadian citizen

  • Valid provincial health card

  • Not previously benefited from Power of Hope program

  • Cancer treatment has not begun

How to Apply

  • Application (mailed/emailed/faxed by the social worker, doctor or nurse)

  • Copies of all bills/rental agreements

  • Brief narrative outlining patient's situation and family's need must be included and written by the social worker

  • Patients with all women's cancers such as breast, cervical, endometrial, ovarian, and uterine cancer

Pink Diamond Women's Cancer Fund

Monthly program that provides for non-medical expenses associated with cancer. Maximum of $500 may be approved per person per year. Only utility bills 30 days overdue and rent payments 60 days overdue will be considered. Cell phone bills are not eligible unless it is the only phone used. Cable payments, mortgage payments, car payments, insurance, medical payments and transportation costs are not eligible for funding.

Kelly Shires Breast Cancer Foundation

Financial assistance for transportation, parking, food costs, home health, prosthetics, supportive care, rent, etc. Patient can apply 5 times to the Trust fund per calendar year with a maximum of $1000 each application.

Helping Hands Fund

Monthly program that works through  social program that provides emergency bridge payments directly to utility companies and landlords.  Maximum of $500 utility and rental bills approved per family per year. Only utility bills 30 days overdue and rent payments 60 days overdue will be considered. Cell phone bills are not eligible unless it is the only phone used.

How to Apply

  • EI application online ASAP after you stop working

  • Medical certificate completed and signed by medical doctor

  • Complete and sign Authorization to Release a Medical Certificate form

Eligibility

  • regular weekly earnings from work have decreased by more than 40% because of providing care for child

  • 600 insured hours of workinthe 52 weeks prior to the start of the claim

  • Child with cancer must be 18 years or younger

Employment Insurance for Parents of Critically Ill Children (PCIC)

55% of your average weekly insurable earnings (up to a maximum amount of $47 400) of up to 35 weeks of benefits during a 52 week PCIC window of when your medical certificate is issued.

Canadian Breast Cancer Support Fund

This fund provides assistance for expenses not covered by health insurance, social assistance and family income. Applicants may apply for financial assistance of $1000 twice a year ($2000 maximum).

POGO Financial Assistance Program

POFAP is a funding program that assists Ontario families with out-of-pocket expenses for accommodation, food, and child care for siblings under the age of 12, when the parent accompanies the child to the hospital during active cancer treatment and care.

 

 

Eligibility

  • All Ontario families with a child in treatment for a malignant disease, who feel that they are in need of financial assistance are eligible. They support Ontario residents who have a valid OHIP number, are diagnosed with cancer before turning 18, and are undergoing active cancer treatment.
